Add Booking Log for Cancelled Appointments

Right now the booking log is available for current and past appointments but not for cancelled appointments. It would be good to see the history log even for cancelled appointments including when the appointment was cancelled.

This is really needed especially when we need to determine when/why an appointment was cancelled and by who.
